The short version

  1. 1

About 1 in 3 Indian adults has fatty liver, including people who don’t drink and people who aren’t overweight.1,2

  2. 2

The ultrasound grade shows how much fat there is, not how much scarring.3

  3. 3

Losing 5% of your weight lowers liver fat. Losing 10% or more can ease scarring too.4

Your scan

What grade 1, 2 and 3 mean

The radiologist grades how bright your liver looks on ultrasound. Fat makes it brighter.

Grade 1 · mild
Slightly brighter than normal. The fine details are still clearly seen.
Grade 2 · moderate
Brighter, and some of the details start to blur.
Grade 3 · severe
Much brighter, and deeper parts of the liver are hard to see.

Scarring is checked differently: first a blood score called FIB-4, then a FibroScan if needed. Ask your doctor which you need.3,5

India

Common, and often missed

38.6%
of Indian adults have fatty liver, in a review of Indian studies.1
1 in 7
Indians with fatty liver were lean, with a BMI under 23, and their liver disease was just as severe.2

What helps most

The weight-loss ladder

Each step does a little more for the liver. For someone weighing 80 kg, the steps are 4 kg, 5.6–8 kg, and 8 kg or more.

  1. Lose 5%

    Less liver fat

The first step, and it counts. Keep it off.

  2. Lose 7–10%

    Less inflammation

The liver starts to settle down.

  3. Lose 10% or more

    Less scarring

Scarring can improve, over a year or more.

What to eat

Fatty liver mein kya khana chahiye?

Guidelines suggest a Mediterranean-style diet. On an Indian plate, that looks like this.

  • Sugary drinks and packaged juiceWater, chaas, plain tea or coffee

Sugary drinks are on the guidelines’ avoid list. Coffee is linked to less liver damage.4,5

  • Biscuits, namkeen and instant noodlesRoasted chana, fruit, a handful of nuts

Cutting ultra-processed food is part of the advice.4

  • Maida and white rice at every mealWhole grains, millets, dal and vegetables

Fibre-rich staples keep you full on fewer calories.

  • Drinks “just on weekends”Less alcohol, or none

No amount of alcohol protects the liver.4

  • Detox drinks and liver tonicsWeight, food and movement

Milk thistle (silymarin) showed no benefit on liver biopsy.5

Move for 150 minutes a week. Exercise lowered liver fat in trials even when people lost little weight.4,7

How long it takes

Fatty liver kitne din me thik hota hai?

There’s no fixed number of days, but here’s what to expect.

Within weeks
Liver fat starts to fall once you eat a little less and move more, steadily.7,8
At 12 weeks
Changes in weight and waist, and often in your SGPT (ALT). Less fat may show on a repeat scan.5
At 12 months
About two in three people in a lifestyle programme no longer had fatty liver.6
A year or more
Scarring improves, mainly with 10% or more weight loss.4,9

Myths

What you might hear

  • They say

    “Only drinkers get fatty liver.”

    Actually

The most common kind is driven by weight, sugar and insulin, not alcohol. About 1 in 3 Indian adults has it.1,10

  • They say

    “Thin people can’t get it.”

    Actually

In an Indian study, 1 in 7 people with fatty liver were lean, with equally severe liver disease.2

  • They say

“My SGPT is normal, so my liver is fine.”

    Actually

A truly healthy ALT is lower than many printed ranges, and ALT can be normal even in advanced disease.5,11

  • They say

    “Grade 1 is nothing.”

    Actually

Ultrasound misses mild fat and can’t see scarring. Fatty liver is also linked to more heart problems.3,4

Get urgent care if you notice

  • Yellow eyes or skin.
  • A swollen tummy or swollen legs.
  • Vomiting blood, or black, tarry stools.
  • Sudden confusion or slurred speech, or bruising easily.

Ask your doctor for a scarring check (FIB-4 or FibroScan) if you have diabetes, belly fat plus another risk such as high BP, or abnormal liver tests.

Fatty liver is usually silent. Some people feel tired or have discomfort under the right ribs.12,13

Questions

Questions people ask

Fatty liver mein kya-kya khana chahiye?

Build your plate around vegetables, dal and legumes, whole grains or millets, nuts and curd, with oil in measured amounts. Avoid sugary drinks and packaged juice, and cut ultra-processed snacks. Plain tea and coffee are fine.

Fatty liver kitne din me thik hota hai?

There is no fixed number of days. Liver fat starts to fall within weeks of eating a little less and moving more. In a trial, about two in three people no longer had fatty liver after 12 months. Scarring takes a year or more and mainly needs 10% weight loss.

What do grades 1, 2 and 3 mean?

They describe how bright the liver looks on ultrasound, which shows how much fat it holds. They don’t show scarring; that needs a FIB-4 score or a FibroScan.

My SGPT is normal. Why do I have fatty liver?

SGPT (ALT) can be normal even when the liver is fatty, and a truly healthy ALT is lower than many labs’ printed limits. The scan and a scarring check tell you more.

Is a little alcohol okay with fatty liver?

Guidelines see no protective amount. Less is better, and none at all if there is scarring.
