HbA1c between 5.7 and 6.4? This is your window.

Prediabetes is common and silent, and small, steady changes can bring it back toward normal. What your number means and what moves it.

A man in his forties reads on his phone at a kitchen table in the morning, a tumbler of chai beside him.

The short version

  1. 1

HbA1c is your average blood sugar over the last 2–3 months. 5.7–6.4% is prediabetes.1

  2. 2

More Indians have prediabetes than diabetes: 15.3% against 11.4% of adults.3

  3. 3

Lifestyle change cut the risk of diabetes by 28.5% in an Indian trial and by 58% in a US one.4,5

Prediabetes

Your window to act. Small, steady changes now can bring it back toward normal. Retest in about 3 months.

Slide to your own number. HbA1c doesn’t need fasting, and it reflects the last 2–3 months.

Fasting sugar

Why two reports can disagree

Indian and American guidelines draw the fasting line in different places.

Normal
Below 100 mg/dL by the American (ADA) cut-off, and below 110 by ICMR’s.
Prediabetes
100–125 by ADA, and 110–125 by ICMR.
Diabetes range
126 or more, confirmed with a second test.

So a fasting sugar of 104 is prediabetes on one report and normal on another. Check your HbA1c too.1,2

India

You’re far from alone

15.3%
of Indian adults have prediabetes, in ICMR’s survey of 1.1 lakh people across every state.3
11.4%
have diabetes. Unlike diabetes, prediabetes wasn’t more common in cities than in villages.3

  • Lifestyle advice39.3%

What to change

Swaps that move HbA1c

  • Fruit juiceWhole fruit: guava, papaya, apple, orange

Whole fruit was linked to slightly lower diabetes risk; juice to higher.2,7

  • Gur or honey in your chaiLess sweetness, a little less each week

Honey and gur count as sugar too.2,6

  • White rice or maida at every mealWhole grains, millets and dal

ICMR’s advice: carbohydrates from whole grains, millets and pulses, in fibre-rich Indian meals.2

  • Sitting straight after dinnerA 10–15 minute walk

Even light walking lowers the rise in sugar after a meal.8

The next 12 weeks

One habit at a time

HbA1c covers about three months, so a 12-week plan shows up on your next report.

  1. Weeks 1–2

    Fix the drinks

No juice or cola; less sugar in chai and coffee.

  2. Weeks 3–6

    Move most days

Walk after meals and build to 150 minutes a week, plus strength exercises twice a week.2

  3. Weeks 7–10

    Fix the plate

More vegetables, dal and millets; less white rice and maida.

  4. Weeks 11–12

    Retest HbA1c

It now reflects your new habits, not the old ones.1

Myths

What you might hear

  • They say

“Gur and honey are fine, they’re natural.”

    Actually

WHO counts honey as a free sugar, and ICMR lists honey among the sweeteners people with diabetes should avoid.2,6

  • They say

    “Fruit is bad for sugar.”

    Actually

ICMR allows guava, papaya, apple, pear and orange in moderation. It’s juice it advises against.2

  • They say

    “Only fat people get diabetes.”

    Actually

Indians carry more body fat at the same weight, which is why ICMR screens from a BMI of 23.2

  • They say

“Borderline means there’s nothing to do.”

    Actually

Without help, more than half of the Indian trial’s borderline group developed diabetes within 3 years.4

See a doctor soon if

  • Your HbA1c is 6.5% or more, or your fasting sugar is 126 or more.
  • You pass urine often and feel thirsty all the time.
  • You’re losing weight without trying, or feel unusually tired.
  • You get repeated urine or genital infections, wounds that heal slowly, or blurred vision.

Go to emergency care for high sugar with vomiting, stomach pain, fast breathing, fruity-smelling breath or confusion.

More than half of people with diabetes have no symptoms at all, which is why the test matters.2,9,10

Questions

Questions people ask

HbA1c kitna hona chahiye?

Without diabetes, below 5.7%. From 5.7 to 6.4% is prediabetes, and 6.5% or more is in the diabetes range and needs a confirming test. If you already have diabetes, your doctor sets your target; ICMR’s ideal is below 7%.

My fasting sugar is 104. Am I prediabetic?

On reports that use the American cut-off of 100, yes. Under ICMR’s cut-off of 110, it is normal. Check your HbA1c and repeat the test.

Can prediabetes go back to normal?

Often, though not always. Lifestyle change cut diabetes risk by 58% in a US trial and by about 28% in an Indian one. Weight loss, daily movement and fewer sugary drinks do most of the work.

Can I eat fruit, gur or honey?

Whole fruit in sensible portions is fine, and guava, papaya, apple and orange are good choices. Juice, gur and honey all count as sugar, so keep them small.

When should I retest HbA1c?

About 3 months after you start changing habits, because HbA1c reflects the last 2–3 months. With prediabetes, test at least once a year.
